The young adult must be employed or enrolled in a training program or school. We are currently seeking full time, part time and per diem nurses to work in our Home Assistance, Adult Day Health and Adult Family Care programs. All positions require a Massachusetts license and experience working with medically/mentally challenged adults.
Our agency has an opening for a permanent, part time Fee for Service Social Worker / Clinician who will be responsible to provide outpatient individual, group and/or family treatment to clients of our Child & Youth Specialty Services Program. The successful candidate must be a license-eligible clinical social worker / clinician with a Masters Degree in Social Work, counseling, psychology, education or rehab counseling from an accredited education institution. VAN DRIVER / PART TIME
Great opportunity for a retiree to supplement their income transporting program participants to and from our After School Day Care sites in accordance with a schedule approved by the Site Coordinator. Also will assist at After School Day Care sites as needed. The successful candidate must possess a high school diploma or equivalent, maintain a valid MA 7D license with good driving record and be able to successfully pass a background check. Must be CPR-certified and pass a drug and alcohol test in addition to being enrolled in a drug consortium for random testing.

CQI DATA SPECIALIST
The Continuous Quality Improvement Data Specialist works with the CQI team to implement quality management and quality improvement activities for programs and the organization as a whole. The CQI team builds improvement activities in response to data findings, trends and expectations established by regulatory and funding agents.
Duties include assisting the Director of Quality Improvement in gathering, reviewing and monitoring data as established by the agency’s Quality Management Plan and designing and implementing appropriate responses to effect lasting improvement.
Candidates must have a minimum of a Bachelor’s Degree in a related field or at least 2 years experience working with quality improvement activities for an agency or corporation. Experience working with outcome data and designing quality improvement activities preferred. Knowledge of JCAHO, CARF or COA accreditation helpful. Excellent computer, organizational, written and oral communication skills are also required.
INTENSIVE CARE COORDINATOR (ICC) - BACHELORS LEVEL
The ICC provides intensive care coordination and case management services to children referred by the five MassHealth Managed Care Entities. The ICC facilitates the development of the Care Planning Team, which utilizes multiple tools, including strength-based assessments, inclusive of CANS, BPRS-C in conjunction with a comprehensive assessment and other clinical information to organize and guide the development of and Individual Care Plan and a risk management / safety plan. Willingness and ability to work flexible hours including evenings, participate in 24 hour on-call emergency pager coverage.
Minimum staff qualifications for a care coordinator include: Bachelor’s level in Social Work, Psychology, Counseling or a related field from an accredited school, an associate’s degree or high school diploma and a minimum of five (5) years of experience working with the target population pursuant to MCE credentialing criteria. Experience needed in navigating any of the child / family-serving systems and experience advocating for family members who are involved with behavioral health systems. They must also have experience working with children and families, be willing to work with Serious Emotional Disturbance population and be able to work flexible hours, including evenings and weekends. They must also have experience in navigating any of the child/family-serving systems and experience advocating for family members who are involved with behavioral health systems. FAMILY PARTNER
The Family Partner is a member of the Community Service Agency staff, serving as a liaison. Family Partners provide involvement, support and advocacy to help the youth and family enrolled in the Intensive Care Coordination and Community Service Agency.
Applicants should have a GED/ High School Diploma with 1 to 2 years experience dealing with children and adolescents or an Associates/Bachelors Degree in the human services field. Partners must have experience as a caregiver of a youth with special needs and preferably a youth with mental health needs. Applicants must have knowledge and experience in navigating child and family-serving systems and be able to teach family members who are involved with child and family-serving systems.
CASE MANAGER – ADULT FAMILY CARE
The Adult Family Care Social Worker / Case Manager evaluates potential participants and foster families. In addition, the AFC Social Worker / Case Manager will monitor placement status and provide case management services and assist in outreach to increase community awareness of program services. Potential applicants should possess a minimum of a BA (MSW is preferred) with prior experience working with the disabled / elderly adults in a community setting.
THERAPEUTIC MENTOR - LIFT PROGRAM
A Therapeutic Mentor provides supportive therapeutic community mentoring services to assigned participants (youth under the age of 21) in any setting where the youth resides. Therapeutic Mentoring offers structured, one-to-one, strength-based support services between a therapeutic mentor and a youth for the purpose of addressing daily living, social, and communication needs. Services include supporting, coaching, and training the youth in age-appropriate behaviors, interpersonal communication, functional skill-building, problem-solving and conflict resolution, and relating appropriately to other youth, as well as adults, in recreational and social activities pursuant to a behavioral health treatment plan developed by an outpatient, or in-home therapy provider in concert with the family, and youth whenever possible, or Individual Care Plan (ICP) for youth in ICC.
These services help to ensure the youth’s success in navigating various social contexts, learning new skills and making functional progress, while the Therapeutic Mentor offers supervision of these interactions and engages the youth in discussions about strategies for effective handling of peer interactions.
A Therapeutic Mentor must have: a good driving record; be 21 years of age or older. A Mentor should have a minimum of High School Diploma/GED or a bachelor’s degree in a human service field from an accredited university and one to two years experience working with children/adolescents/transition age youth.
